Use this form for billing questions, profile corrections or removal, withdrawn consent, impersonation, harassment, financial pressure, or another safety concern. For a profile correction, include the approved display name and the corrected facility or institutional mailing details. An administrator reviews every request before changing a profile. Facility details are kept private and are not displayed on the public profile.

To protect the form from spam, each network can send up to 20 requests every 10 minutes, and each email address can send up to 3 requests per day. These limits apply only to support requests.
